In Rockville, a Washington D.C. suburb located in Montgomery County, MD, the costs of senior care are among the highest in the state. The 2018 Genworth Cost of Care Survey revealed that nursing home care and adult day care services are particularly expensive in this area with costs that can exceed the national average by thousands of dollars per month. For example, the average cost of nursing home care in the area is $10,539, while the national average is just $7,441 — a difference of more than $3,000.

Assisted living costs in Rockville average $5,000, which is $327 more than the state average of $4,673. Of the metropolitan areas surveyed, only Baltimore and Cumberland offer more affordable pricing, with costs that hit $4,250 and $3,650, respectively. Hagerstown is comparable at just $45 more than seniors can expect to pay in Rockville, while the California area tops the list for most expensive at $900 more than in Rockville.
Home health care costs in Rockville are the most expensive in the state, but the average cost is $4,290 — just $193 more than the state average. Baltimore costs are comparable at $4,004, while seniors can expect to save $858 on services delivered in Cumberland. In most areas in Maryland, seniors can expect to pay between $3,800 to just over $4,000 per month, based on an average of 44 hours on in-home service per week.
In Rockville, adult day care services are the most expensive in the state at $1,972, $239 more than the state average of $1,733. Baltimore is comparable to Rockville, with an average price of $1,880, just $92 less per month.
Less expensive areas like Salisbury and Hagerstown fall between $291 and $260 less than in Rockville, while the California area is in line with the state average. Cumberland seniors can expect to pay $199 less per month with an average cost of $1,773.
Nursing home care in Rockville costs an average of $10,539 per month. This is $1,308 more than the state average and the highest price point in the state. Baltimore and Salisbury tie for second most expensive with an average cost of $9,733, $806 less than seniors might expect to pay in Rockville.
Several areas are less expensive than Rockville, with California showing the most potential savings at an average cost of $7,756. Cumberland is comparable to California and $2,631 less than in Rockville.
The state-run Medicaid program in Maryland is known as Medical Assistance (MA). MA participants enroll in an entitlement program, which means all participants are eligible to receive covered services. For seniors who need nursing home level care, MA does offer assistance. Under Medicaid waiver programs, financial assistance may be available for in-home health services or daytime care options.
Qualifying for Medical Assistance can be complicated, but there are some fairly solid income guidelines that apply. Single senior applicants must earn no more than $2,313 per month. This is equal to $300 of the Federal Benefit Rate (FBR) and may change every year along with the FBR. Along with the income limits comes asset restrictions. There may also be exclusions for burial plots, life insurance or certain types of trust accounts. Married couples with only one spouse applying face different asset restrictions. The non-applicant spouse may retain up to $3,160.50 and assets valued at up to $126,400.
For frail or disabled seniors requiring an institutional level of care, there is the Community Personal Assistance Services program. Participants receive financial assistance to pay for a home health aid that assists with the activities of daily living. This may include help with bathing, grooming, preparing meals, medication monitoring, supervision, or other services that can help a senior extend their stay in their own home.
To qualify for the Community Personal Assistance Services program, seniors must be Medicaid eligible. Seniors receiving SSI or Temporary Cash Assistance automatically qualify for Medicaid, offering a shortcut to enrollment. This particular program also has functional requirements. Seniors must have a disability or need assistance with activities of daily life that compromise their ability to live independently.
Once enrolled in the program, seniors have the option of selecting a care provider or allowing the program administrator to make the selection. Service options include a variety of assistance services including help getting dressed, meal preparation or help around the house. This program allows seniors to select a care provider, including friends or family members. Care providers are paid at a rate of between $12.75 to $16.75 per hour for the number of hours covered by the program. In addition to these services, seniors may also get financial assistance paying for the installation and monitoring of an emergency response system or other assistive technology. Community Options Waiver
The Community Options Waiver offers services to seniors who would otherwise need nursing home care. It is designed to help seniors age in place, though it may also fund some of the costs associated with assisted living or a move to a senior group home. Only assisted living facilities and group homes that accept Medicaid reimbursements are eligible options under the Community Options Waiver. In a home or independent living setting, this waiver may help with meal delivery, adult day care, home mobility modifications or other in-home ADL assistance. This is not an entitlement program and there are enrollment caps. This means seniors may be waitlisted pending an opening, so an early application is best.

Community Pathways Medicaid Waiver
The Community Pathways Waiver is designed to help seniors with a chronic intellectual or developmental disability, like Alzheimer's or dementia. For qualified seniors who are able to manage a service provider, self-directed care is an option. This allows family members to act as the primary caregiver for participants. Under this program, seniors may be eligible to hire a home health aide, enroll in adult day care or participate in respite care designed to offer caregivers a break.

Medical Day Care Services Waiver
The Medical Day Care Services Waiver offers an alternative to nursing home placement for seniors at risk of transferring on a long-term basis. This waiver allows seniors to spend their days at a licensed adult day care facility and live with friends or family during the evenings. Daycare services offer social activities, medical care, meals and other ADL assistance.
While this program is available to all eligible individuals aged 16 or older, there may be waiting lists for enrollment caps. Qualified seniors earn no more than the current SSI benefit, though there may be alternative paths to qualification for those deemed medically needy. Seniors already in a long-term nursing home placement may get help moving back home or returning to the community with the Increased Community Services Waiver. This program helps fund home modifications and assistive technologies designed to make living at home a safe and sustainable option for some seniors, particularly with covered personal care services available on an ongoing basis.
Qualified applicants earn no more than 300% of the FBR, the same restrictions that apply to Medicaid. The cost of in-home care services must be lower than the delivery of the same services in a nursing home setting. Senior Care Services Program
The Senior Care Services Program is available to eligible seniors in Rockville, though it is not an entitlement program and waiting lists may apply. Participants in this program may receive assistance with personal care like housekeeping, bathing and grooming, or transportation, chore help and respite care.

The Congregate Housing Services Program is designed for those seniors considering a group home setting. It is available in six Maryland Counties, including Montgomery County. Seniors in a group home usually receive help with bathing and dressing, meal preparation, medication monitoring, and chore help. Qualified applicants earn no more than 60% of the area median income and own no more than $27,735 in countable assets. In Rockville, the In-Home Aide Services Program may be available to seniors at high risk of a nursing home transfer. Seniors facing a nursing home placement get priority placement on the waiting list. Unlike many available assistance programs, there is no income limit on the IHAS program. Instead, services are offered on a sliding scale, with costs adjusted based on a senior's ability to pay.
