Program Description

HoosierRx is Indiana’s state pharmaceutical assistance program. It provides financial assistance to help seniors pay their Medicare Part D premiums, which provides prescription drug coverage. It also helps persons who have a Medicare Advantage plan cover the cost of the prescription drug portion of coverage.

This program is specifically designed for low-income Indiana residents. By providing financial assistance to enable seniors to get their basic medications, the program’s aim is to prevent more complicated medical conditions.

A number of companies are currently working with HoosierRx. These companies include AARP/United HealthCare, CIGNA, EnvisionRx/ Elixir, Aetna/SilverScript, and WellCare.

Eligibility Guidelines

To be eligible for HoosierRx, seniors must meet the following criteria:

General Requirements

  • Be an Indiana resident. There is no minimum length of residency requirement.
  • Be age 65 or older.
  • Be enrolled or enroll in a Medicare Part D plan or Medicare Advantage plan with one of the following companies: AARP/United HealthCare, CIGNA, EnvisionRx/ Elixir, Aetna/SilverScript and WellCare.
  • Have applied for Medicare Extra Help via the Social Security Administration and received either a notice of denial or award.

Financial Requirements

To be eligible for HoosierRx, an applicant’s income must be under 150% of the Federal Poverty Level plus $240 a year. (An extra income of $20 per month is disregarded.) For 2023, this equates to a max of $22,110 a year if single or $29,820 a year if married.

Individuals who are enrolled in full Medicaid are ineligible for HoosierRx. 

Benefits and Services

HoosierRX offers assistance with Medicare Part D premiums, or the prescription drug coverage portion of Medicare Advantage. Participants can receive up to $70 per month. There are no costs to enroll in this program.

To apply for HoosierRx, residents should call HoosierRx at 866-267-4679. One can also apply for the program by downloading an application. Completed applications can be faxed to 317-234-3709, or mailed to:

Candidates should be aware that, if they have not already done so, they are required to apply for Medicare Extra Help prior to being accepted into HoosierRx. A copy of the applicant’s Notice of Award or Notice of Denial should be submitted with the application, as well as proof of income.
