Important Note: This waiver was a pilot program tested in 4 Florida counties. It was created to determine if a disease specific waiver could provide better services at a lower cost and delay or prevent nursing home placement. After several years of study, this waiver was found to be no more effective than other Medicaid HCBS waivers. The Legislature permitted it to expire in 2010.
Florida residents with Alzheimer's or dementia may receive services under the Statewide Managed Long Term Care or the Florida Alzheimer's Disease Initiative.

This Medicaid home and community based services waiver in Florida is specially designed to provide care for individuals with Alzheimer’s disease in the community. The ADW helps keep patients at home longer with their loved ones enhancing their quality of life and avoiding the costs of nursing home placement.
In addition to being diagnosed with Alzheimer’s disease, individuals must be over 60 and live at home or with a qualified caregiver and be financially qualified for Medicaid.
The Alzheimer's Disease Waiver program is not available. It was only a pilot available in four Counties: Broward, Miami-Dade, Palm Beach and Pinellas. Now, individuals suffering from Alzheimer's Disease or another type of dementia and cannot afford their care should apply to the Florida State Medicaid Managed Care program for long term care services and supports at home or in the community.
