New Jersey Assistance for Community Caregiving (JACC)

 What is New Jersey Assistance for Community Caregiving?
New Jersey Assistance for Community Caregiving, or JACC for short, is a non-Medicaid program that provides a variety of care services and supports to elderly residents that require assistance with their activities of daily living and are at-risk for nursing home placement.  Services are also provided to the primary caregivers of qualified participants in the form of respite care.

This program has a Participant-Employed Provider service option, sometimes referred to as Cash and Counseling or Consumer Direction.  This means that program participants have the option of choosing their own service and care providers rather than have those providers chosen by the state on their behalf.  Friends, relatives and certain family members can be hired as paid caregivers.
 What are the Eligibility Requirements for the NJ JACC Program?
In addition to being New Jersey residents and at least 60 years of age, there are financial and functionality requirements to be eligible for this program.  Functionally, individuals must require the level of care provided in nursing home however they must live at home, in a rental property or with family members.  They cannot live in a residential care facility such as adult foster care or assisted living.

The income limit is 365% of the Federal Poverty Level, or approximately $3,100 / month. Their assets must be valued at less than $40,000 for an individual or $60,000 for a couple.  However their income and assets cannot be so low that they are eligible for and receiving care from Medicaid or under a Medicaid Waiver. 

 What Services are Provided in the NJ JACC Program?
A broad array of services and supports are available in the JACC Program.  Participants and their caregivers can receive any of the following:

-Adult day care and adult day health care
-Caregiver training
-Chore services such as yard work and light home maintenance
-Durable medical equipment and supplies
-Home modifications to improve accessibility
-Home-delivered meal service
-Homemaker services such as laundry and housecleaning
-Personal care
-Personal emergency response service
-Respite care for the participant's primary caregiver
-Transportation assistance for medical appointments
