Illinois Community Care Program

 What is the Illinois Community Care Program?
Illinois Community Care is a program intended to allow seniors who are at risk for nursing home placement to remain living at home or the home of a caregiver and receive services and care at those locations.  This program is designed to provide increased flexibility to care recipients and cost savings to the state by preventing the premature or unnecessary institutionalization of elderly state residents.

What are the Eligibility Criteria for the Illinois Community Care Program?
U.S. citizens or legal aliens who are 60+ years old and reside in Illinois are eligible if they have non-exempt assets of $17,500 or less (Non-exempt assets do not include the home, car, or personal furnishings.). In addition, seniors must be assessed as having a need for long term care.

Income level is not a eligibility factor, but it is considered in the cost of services. Those seniors whose income is below the federal poverty line receive services free of charge. For those whose income exceeds the federal poverty line, fees are determined by a variety of factors.

 What Benefits are Provided in the Illinois Community Care Program?
This program provides services and supports to the elderly in their homes or communities which include the following:

-Adult day care
-Case management
-Homemaker / chore services
-Personal care
-Senior companion
